Follow the instructions below to register, submit a Preliminary Registration Form, evaluate subjects, and manage the Online PRF System.

Student User Guide

Only approved student accounts can log in and submit a PRF.

Create a Student Account

From the login page, click Create Student Account. Enter your student ID, full name, email, username, and password.

Wait for Administrator Approval

After registration, your account status is pending. You cannot log in until the administrator approves the account.

Log In

Enter your approved username and password on the login page.

Create a New PRF

From the Student Dashboard, click Create New PRF.

Complete the PRF

Fill in the required personal and academic information. Check the student confirmation box to apply your electronic signature.

Submit for Evaluation

Click Submit PRF for Evaluation. The form will be forwarded to the assigned evaluator.

View Evaluated Subjects

Return to the Student Dashboard and open your PRF. Once evaluated, the recommended subjects and total units will be displayed.

A student can create only one PRF for the same semester and academic year.

Student PRF Status

Status Meaning
For Evaluation The PRF was submitted and is waiting for evaluator review.
Evaluated The evaluator has added the recommended subjects and total units.
Printed The evaluator has opened the official printable PRF.

Evaluator User Guide

Log In as Evaluator

Enter your evaluator username and password.

Open a Student PRF

From the Evaluator Dashboard, locate the student's submission and click Evaluate or View/Edit.

Enter the Subject Code

Type a valid subject code from the subject database. The description and number of units will appear automatically.

Add Optional Subject Information

Time, days, room number, and instructor may be entered when available. These fields may be left blank.

Check Total Units

The system automatically calculates the total number of units for all selected subjects.

Save the Evaluation

Click Save Evaluation. The PRF status will change to Evaluated.

Print the PRF

Click Print PRF. If the PRF has already been printed, use Print Again.

Before saving, make sure every subject code is valid and the description and units were loaded from the database.

Administrator User Guide

Log In as Administrator

Enter the administrator username and password.

Review Pending Accounts

Open the pending student account list and verify the student's ID, name, username, and email.

Approve or Reject Accounts

Click Approve to activate a valid student account. Click Reject for invalid or duplicate registrations.

Add Subjects

Open Manage Subjects. Enter the subject code, description, and units, then save the record.

Edit a Subject

Click Edit beside a subject to update its code, description, units, or status.

Activate or Deactivate Subjects

Deactivated subjects cannot be selected by the evaluator but remain available in previous PRF records.

Do not create duplicate subject codes. The subject code is used by the evaluator to retrieve the correct description and units.

Frequently Asked Questions

Why can't I log in after creating an account?
Your student account must first be approved by the system administrator.
Why can't I create another PRF?
The system allows only one PRF per student for the same semester and academic year.
Why is the subject description not appearing?
Check that the subject code is correct and that the subject is active in the administrator's subject database.
Can an evaluator print the PRF again?
Yes. Open the evaluated PRF and click Print Again.
Why is my electronic signature not showing?
Make sure the student checked the electronic-signature confirmation before submitting the PRF.
